Mr - I’d like to register today - and I’d like to arrange an appointment to see a Dr as soon as possible.

R - ok no problem I just have a few questions to ask you so I can get you set up on our system

- address ?

Mr - blarr blarrr

R - ok we just also need to know the name and address of your previous Doctors

Mr - oh I can remember that

R - what do you mean can’t remember

Mr - it means I can’t remember

R - so no idea his or her name or address ?

Mr - usually a different Dr each time , you know how it goes. It’s been a while since I went last ...when I lived there , and now I moved to this area all I just can't remember,  it was in Wimbledon ,,,somewhere

R - so no idea of name ?

Mr - No

R - no idea of the street ?

Mr - no , it must have been at least 5 or 10 years ago if not more  

R - I’m not sure they will accept this

Mr - who are ' they '...can I speak to ' they ' to explain my situation ?

R - the practice whoever is in charge...the NHS  

Mr - why

R - well you need records history ' I ' have to complete your details and register you onto ' our ' system.

Mr - I'm sure ' they ' or I was hoping you , could key in my details and something will come up

R - they always ask for previous GP's name address and so on 

Mr - I'm sure I dont have details of my ' so on ' ....what if I have just arrived in the country

R - have you

Mr - no

R- well why say that, what difference would it make ?

Mr - if for example I arrive as a refugee from a war-torn country , maybe my previous GP no longer exists all records gone ...what would happen then ? how would ' they ' deal with me ?

R- but you haven't have you , you already told me he was based in Wimbledon.

Mr - I think , might have been I can't remember ...but there must be a process for thickos like me?

R - the process is you tell me it’s not so difficult you must know the area , street , do you have any papers past appointment papers anything you can go home and look through.

Mr - I thought this would be easier, I don't think I have anything at home , I can't remember having any appointment papers.

R - nothing at all ?

Mr - not that I can remember

R - well I can't complete your application that's that

Mr - I thought you might be more helpful suggest something ? I really need to see a Dr today

R - well what seems to be the issue that ' so so ' important

Mr - I have amnesia
