Perhaps three types of pains and pleasures exist in living beings,,
At physical, mental or Intellectial level they create the feelings.
Pain or pleasure, sweet or bitter as felt by five organs of senses,
are examples under the physical level in the above classes.
Worries, anxieties and frustration may come under mental turmoil and tension,
Insult and injury against one’s ego could come under intellectual humiliation.
How are these feelings transmitted and which part of a being is the perceiver,
What is the total mechanism? The author is searching for an answer.

I enjoyed this particular piece, as it is very relevant of late, in the mindset of someone in my life I completely adore.
My take on your question is:-
The feelings are triggered by chemical response of the brain, if the scientists had their say...then transmitted from some lobe or other deep inside that same cavernous place of agony and ecstasy...brain matter.
However, us writers with our prose and nuances would probably agree with the comment already received from Highhat. It's got to be the uncharted depths of the soul and will only ever be confirmed by the perceived as their life on Earth reaches it's final moment.
